Printing Medical Release Forms
AYSO requires that coaches must have printed copies of “Player Application Forms” (aka Medical Releases) on hand for any game or practice. Coaches can download these forms (which are already filled-out from the players registration) by following the instructions below.
1. Login to the Association Platform
These forms are accessed via the “Association Platform”, which you can access via this URL:
The login credentials for the Association Platform should be the same as you currently use when logging in to your account at ayso109.org.
2. Open Your Team
This will take you to a page that shows teams to which you are assigned as a coach. You can then click “Team Info” to view the team you want to print the medical release forms for.
3. Select Players and Download
Select the checkbox next to each player you want to print forms for (you can select the column header to select all players).
From the “Print” menu in the lower right-hand corner select the “Player Application Forms” option, which will then download a PDF with the forms to your computer.
The forms are already filled-out and digitally signed from the player’s registration, so all you need to do is p
rint them and keep copies with you whenever you’re conducting a game or practice.
