Game cards

At any AYSO game, each team will give the referee (or the assistants) an Official AYSO Lineup Form which gives the team name and number, coaches names and the name of each player. This card has space on it to record which players are not playing each quarter, and is thus essential to help you monitor and enforce AYSO's "everyone plays" policy.

However, the card does not have much free space on it, so it is generally difficult for a referee to use it to keep a record of the game. Some try, and just scribble over the pre-printed text areas. Others just stick the lineup card in a back pocket and never look at it except at substitution times and game end. If you are one of the latter, you must use something else to keep your game record. You have a few choices

Or you could download GameData.PDF, which contains a card layout (in PDF format) that some have found useful, and either print it or use it to inspire your own design. (If you don't have it already, you'll need to download Adobe Acrobat to view and print this file.)

This card was designed by Roy Levin to whom compliments and suggestions should be directed. We would also be happy to post other designs here for people to try. Email them to us!
